Steel doors do not fail gently, and their frames fail first
Hollow metal openings are the back doors, stairwell doors, mechanical rooms, and service entrances of nearly every commercial building in the metro. They are built to be abused and mostly succeed, which means that when one does fail, it has usually been failing quietly for years.
The two classic Charlotte failures are the bottom of the door and the bottom of the frame. Water sits at the threshold, salt and cleaning chemicals accelerate it, and the steel goes from sound to perforated with very little warning in between.
